Home Care Services

The Home Care Services Program is licensed as a provider of Class A and Class F Homecare.  This allows the program to serve individuals in Housing-with-Services settings such as The Maples at St. John, Vista Ridge Memory Care Suites,  and Cottonwood Ridge, or individuals in their private homes or apartments throughout the community.

The intermittent services that Homecare provides are intended to allow clients to live in the least restrictive setting possible.

Licensed Nurses Can Provide:

  • Medication set-up and management
  • Assessments and care planning
  • Physician updates
  • Other intermittent skilled services
  • Home Care Staff Can Provide:
  • Health Related Support Services
  • Assistance with dressing, grooming and bathing
  • Personal Safety Contacts
  • Socialization
  • Coordination of care
  • Health Care Services
  • RN Delegated Services
  • Medication Administration
  • Blood Glucose Testing
  • TED Stocking Application

And others

  • Meal Preparation
  • RN Trained and Supervised Staff
  • Home Management Services
  • Household chores contributing to a clean, safe living environment, such as:
  • Light Housekeeping
  • Laundry
  • Snack Preparation
  • Shopping

The program does not provide room and board and does not pay raw food costs, the cost of laundry or housekeeping supplies, or the cost of operating laundry equipment. The program does not provide transportation.

Personal Foot Care Visits can include:

  • Foot Soak
  • Nail Trimming
  • Pare Corns
  • Lotion Massage

Homemaker Services:
Homemaker services are available to individuals living in Springfield.  Services may be provided outside of Springfield depending on the individual’s needs, and the overall availability of homecare staff.  Homemaker services are provided in accordance with a Plan for Services that is pre-arranged.  A Registered Nurse completes an evaluation visit to determine the appropriateness of the level of assistance.

  • Light Housekeeping  includes vacuuming, cleaning of sinks, bathrooms, toilets, dusting, trash removal
  • Moderate Housekeeping, includes changing of linens, window washing, refrigerator cleaning, cleaningof assistive devices, wiping cupboards, in addition to all light housekeeping services
  • Laundry Assistance
  • Miscellaneous Assistance including:  reading mail, assistance with ordering groceries.
  • Transportation:  Staff may transport clients to and from appointments.  Availability of this service is based on the overall mobility of the client, assessed by a Registered Nurse.

Homemaker staff are unlicensed personnel, and cannot perform any personal care or hygiene related services.  Home Health Aides may assist with bathing, hygiene, and other delegated services based on the clients plan of care.

Licensure:
Class A and Class F Home Care Provider licensed by the Minnesota Department of Health.

Homecare Services is a contracted provider of Brown County Family Services, allowing income eligible clients to utilize the Elderly Waiver program as payment for services.  Please contact Brown County Family Services for referral information.
